Requirement 1
Complete the online Background Check.
• In the Open the Site Search box type Queen-Saints School if you have children in the school, Queen-Saints otherwise. The access code is protection.
• Be sure to choose volunteer and not employee. Employees require fingerprinting and changing status is difficult.
• Your application is complete when you see 10 green check marks and no red X's.
Requirement 2
Print read and sign. Please make every effort to use QAS parishioners as references.

• Please return this form to the Religious Ed office and not DCFS as stated on the form.
• This form must be submitted annually.
Register for a Virtus training session here.
Requirement 5
QAS Virtus training sessiosn will be offered September 23rd, 2010 and October 14th 2010, but you do not need to attend a session held at QAS. Check the webpage for different times and locations.
All forms should be submitted to the Religious Ed office either through the school or the rectory mailbox. Please include a copy of the VIRTUS training certificate and a copy (screenshot) of the completed online process (10 green check marks page).
